К настоящему времени существуют убедительные данные в пользу иммуновоспалительной гипотезы этиопатогенеза шизофрении. В качестве маркеров иммуно-воспалительных нарушений при шизофрении в небольшом ряде работ изучались гематологические показатели (ГП) и коэффициенты системного воспаления (КСВ): нейтрофильно-лимфоцитарное (NLR), моноцитарно-лимфоцитарное (MLR) и тромбоцитарно-лимфоцитарное (PLR) соотношения. Учитывая, что ГП гипотетически могут отражать иммуно-воспалительные процессы при шизофрении, актуальной является оценка их взаимосвязи с другими известными биохимическими маркерами воспаления и окислительного стресса (ОС) при этом заболевании. Целью данного исследования была оценка ассоциации ГП с биохимическими маркерами воспаления и ОС при шизофрении, а также с выраженностью клинических симптомов. Материалы и методы: Обследовано 50 пациентов с шизофренией: 42 женщины, 8 мужчин, возраст — 36 [<xref ref-type="bibr" rid="cit7">7</xref>] лет. Клиническая оценка проводилась с помощью Шкалы для оценки позитивных и негативных синдромов шизофрении и Шкалы Калгари для оценки депрессии при шизофрении. Изучались ГП, гомоцистеин (Гц), тетрагидробиотерин (ВН4), восстановленный глутатион (GSH), интерлейкин-6 (IL-6), С-реактивный белок (СРБ) и фактор некроза опухоли-альфа (TNF-a). Результаты: Уровень СРБ сыворотки значимо коррелирует с количеством лейкоцитов, суммарным показателем количества моноцитов, базофилов и эозинофилов, а также количеством гранулоцитов, но не с КСВ (NLP, MLR и PLR). TNF-a имеет более тесные прямые корреляции с целым рядом ГП (количество гранулоцитов и эритроцитов, на уровне тенденции к значимости — количество лейкоцитов) и КСВ (NLP, на уровне тенденции — MLR). Выводы: ГП и КСВ в изученной выборке пациентов с шизофренией соотносятся с данными, полученными зарубежными авторами, что свидетельствует о перспективности изучения данной темы в Российской популяции. Актуальной представляется дальнейшая оценка взаимосвязи ГП (КСВ) с TNF-a сыворотки. Изучение ассоциаций биохимических иммуно-воспалительных маркеров с КСВ в дальнейшем требует тщательной оценки стадии развития заболевания, длительности его течения, особенностей ответа на терапию.</p></abstract><trans-abstract xml:lang="en"><p>Relevance. To date, there is convincing evidence in favour of the immuno-inflammatory hypothesis of the etiopathogenesis of schizophrenia. As markers of immuno-inflammatory disorders in schizophrenia, a small number of studies have examined haematological indices (HI) and systemic inflammation coefficients (SIC): neutrophil-lymphocyte ratio (NLR), monocyte-lymphocyte ratio (MLR) and platelet-lymphocyte ratio (PLR). Considering that HI may hypothetically reflect immuno-inflammatory processes in schizophrenia, it is relevant to assess their relationship with other known biochemical markers of inflammation and oxidative stress in this disease. The aim of this study was to evaluate the association of HI with biochemical markers of inflammation and oxidative stress in schizophrenia, as well as with the severity of clinical symptoms. Materials and methods: 50 patients with schizophrenia were examined: 42 women, 8 men, age 36 [<xref ref-type="bibr" rid="cit7">7</xref>] years (median and interquartile range, hereafter Me [Q1; Q3]). Clinical assessment was performed using the Schizophrenia Positive and Negative Syndrome Scale and the Calgary Depression Scale for Schizophrenia. HI, homocysteine (Hc), tetrahydrobioterine (BH4), reduced glutathione (GSH), interleukin-6 (IL-6), C-reactive protein (CRP) and tumour necrosis factor-alpha (TNF-a) were studied. Results: Serum CRP levels were significantly correlated with leukocyte count, total monocyte, basophil and eosinophil counts, and granulocyte counts, but not with SIC (NLP, MLR and PLR). TNF-a has stronger direct correlations with a number of HI (granulocyte and erythrocyte counts, leukocyte counts at the level of trend towards significance) and coefficients (NLP, MLR at the level of trend). Conclusions: HI in the studied sample of patients with schizophrenia correlate with the data obtained by foreign authors, which indicates the prospect of studying this topic in the Russian population. Further evaluation of the relationship between HI (SIC) and serum TNF-a seems relevant.
