Цель Клинико-катамнестическая верификация аттенуированных симптомов шизофренического спектра (АСШС) в структуре первого депрессивного эпизода юношеского возраста как ранних маркеров шизофренического процесса с установлением дальнейших вариантов течения заболевания и его исходов.

Материалы и методы

Материалы и методы. Обследовано 124 больных юношеского возраста (средний возраст 19,6±2,3 лет), впервые госпитализированных в клинику с первым депрессивным эпизодом с АСШС. Группу контроля составили 27 больных с юношескими депрессиями без АСШС. Все пациенты были прослежены катамнестически не менее 5 лет. Средний срок катамнеза составил 7,1±1,6 лет. Для оценки симптоматических и функциональных исходов применялись шкалы HDRS, SOPS, SANS и PSP. Статистический анализ полученных данных проводился с использованием программы STATISTICA 12.

Результаты

Результаты. Разработана типология юношеских депрессий с АСШС с выделением трех основных типов: (1) с аттенуированными позитивными симптомами (АПС), (2) с аттенуироваными негативными симптомами (АНС) и (3) с аттенуированными симптомами дезорганизации. Показано, что юношеские депрессии с АСШС по сравнению с контрольной группой чаще переходят в хронические формы, имеют достоверно худшие функциональные и симптоматические исходы и в большей степени ассоциированы с диагностикой заболеваний шизофренического спектра, причем. как вялотекущих, так и приступных форм шизофрении, на пятилетний катамнез.

Заключение

Заключение. Выделенные симптомы шизофренического спектра в структуре юношеских депрессий имеют высокое сродство между собой, что свидетельствует о едином патогенетическом механизме их формирования, а также обладают предикторным значением в качестве факторов риска развития шизофрении.

Objective Clinical and follow-up verification of Attenuated Schizophrenic Symptoms (ASS) in the first youth depressive episode as early markers of the schizophrenic process, establishing further variants of the course of the disorder and its outcomes.

Materials and methods

Materials and methods. 124 young inpatients (averaged age 19,6±2,3 years) with the first depressive episode with ASS were examined. The control group consisted of 27 patients with youth depression without ASS. All patients have been tracked for at least five years. The average follow-up period was 7,1±1,6 years. The HDRS, SOPS, SANS and PSP scales were used to assess the symptomatic and functional outcomes. Statistical analysis was carried out using STATISTICA 12.

Results

Results. The typological classification of youth depressions (ASD) with ASS has been developed with the identification of three main types: (1) with attenuated positive symptoms (APS), (2) with attenuated negative symptoms (ANS), and (3) with attenuated symptoms of disorganization. Youth depression with ASS, compared to the control group, is more likely to move into chronic forms, has reliably worse functional and symptomatic outcomes, and is more associated with the diagnosis of schizophrenic spectrum disorders at five years follow-up.

Conclusion

Conclusion. Attenuated schizophrenic symptoms in the structure of youth depressions have high affinity to each other, indicating a common pathogenic mechanism of their formation, and also have predicate value as risk factors for schizophrenia.
