В статье, с позиции биопсихосоциальной концепции, представлен опыт деятельности поли-профессиональных бригад по комплексной клинико-психосоциальной реабилитации пациентов с применением инструментария в форме индивидуальной карты. В карте представлены разделы, отражающие реабилитационную диагностику, план мероприятий, применяемые методики, алгоритмы исполнения, результаты. В статье описывается характер взаимодействия специалистов в команде, мониторинг качества деятельности. Делаются выводы об эффективности и положительных результатах применяемой методики.
In the article, with the position of the biopsychosocial concept, presented the experience of the poliprofessional'nyh brigades integrated clinical and psychosocial rehabilitation of patients with the use of tools in the form of individual cards. Topics map reflecting the rehabilitative diagnosis, action plan, applied methods, algorithms, performance results. This article describes the nature of the interaction between specialists in the team, monitoring the quality of activities. Draws conclusions about the effectiveness and positive results of the applied methodology.
