Abstract
Delusion is the most difficult problem in psychopathology. In the existing definitions of delusion there is no clear indication of the discriminant that enables us to reliably distinguish it from other psychopathologic phenomena. This paper proposes an approach to examine delusion as semiotic object that has a symbolic nature. The paper accentuates that a person, along with the biological means of processing and storage of information uses natural and artificial sign systems. Self reflection in our consciousness of objective reality is also an information process with sign-symbolic structure. Therefore, the use of ideas and methods of modern semiotics becomes the conceptual framework that can expand our understanding of the psychopathology of delusion. Destruction of semiosis, ie, the process of generating value, apparently is that discriminant, which allows us to distinguish delusion from other similar phenomena, for example, from overvalued ideas.

Abstract
The article provides an overview of data in the literature devoted to the combined use of electroconvulsive therapy (ECT) and antipsychotics in treatment of resistant schizophrenia. ECT with anesthesia and muscle relaxants is a fairly safe method. The combination of ECT and antipsychotics, in the opinion of many authors, may reduce the symptoms of schizophrenia resistant to antipsychotic monotherapy. There is evidence of the successful combination of ECT with both conventional and atypical antipsychotics. Today the main drug having proven efficacy in treatment-resistant schizophrenia is clozapine. ECT&clozapine in treatment-resistant schizophrenia allows to achieve deeper reduction of symptoms and more rapid onset of therapeutic effect. There is evidence of the appropriateness of maintenance ECT in conjunction with antipsychotic therapy to preserve the achieved effect.

Abstract
Clinical and neurophysiological studies was conducted in 426 patients with epilepsy with longterm (more than 8 years) remission of seizures. It is established that the control of seizures accompanied by epileptic activity regression on EEG to full health compensation for 5-8 years at 45% of the observed, that allows to solve questions of completion of treatment.

Abstract
Inspite of the problems of sigma being widely discussed, there still continue the attempts to understand the ways and factors producing the self-stigmatizing and mechanisms obtruding the involvement of self-stigmatized patients into the treatment process. In the present study there took part 66 in-patients with severe mental disorders. There were used the questionnaire of self-stigmatizing, devaluation/discrimination part of the experienced stigmatizing questionnaire and motivation for treatment rating scale. Detected some significant factors in producing the self-stigmatizing and influence of the implicit self-stigmatizing, the severeness of which results in the development of the explicit self-stigmatization, which destruct the motivation for treatment.

Abstract
The article presents the results of a study on the application possibilities developed by the authors method of «intrapsychic interview» carried out in the structure of the psychiatric examination of patients with depressive episodes of moderate and mild degree. In the comparison group were healthy individuals, who were also conducting «intrapsychic interview». The study involved 64 patients with an established diagnosis of depressive episode (F31.3, F32.0, F32.1) and 23 healthy mental attitude of a person. The study revealed that patients with depressive symptoms experience mental phenomena is characterized by the tendency to avoid experiencing negative feelings, the desire to achieve a state of «inner peace», a tendency to postpone the satisfaction of natural needs, a decline in the ability to recognize their own feelings and their interpretation. According to the study it is assumed that the «intrapsychic interview» allows you to achieve a more complete understanding of the clinical picture of mental disorders and their dynamics.

Abstract
The frequency and expected mechanisms of comorbidity of hyperkinetic disorder with dyslasia, dyslexia, dysgraphia and dyscalculia are considered in the article. The data obtained underline the heterogeneity and complexity of the pathogenetic mechanisms of hyperkinetic disorder, where violations of oral and written speech, as well as dyscalculia, are associated with different pathogenesis links, forming a pathogenetically heterogeneous population. Obviously, the role of the dopamine system in the formation of concomitant specific disorders in the development of speech and language (dyslalia) and specific disorders in the development of educational skills (dyslexia, dysgraphia, dyscalculia) is exaggerated and require additional studies on the pathogenetic mechanisms, taking into account the high level of comorbidity with hyperkinetic disorder.

Abstract
The article address the issue of the need for comprehensive psychological assessment of patients needed to characterize «psychological diagnosis», which (along with the biological, social and functional), an important component of a holistic (holistic) understanding of the nature of mental disorders. The most important psychological characteristics in the framework of psychological diagnosis are: personal characteristics; especially cognitive processes; especially protective and coping; features an internal picture of disease (FIPD), which are discussed in terms of the concept of «psychological adaptation-compensatory potential». The latter provides for the analysis of the characteristics that represent the structure of the individual, including the study of the characteristics of their relations among themselves and with external factors. The aim of the research was to determine the clinical, psychological and experimental psychological features of patients with schizophrenia spectrum disorders. It is concluded that patients with paranoid schizophrenia and schizotypal disorder present disturbances of thinking in the form of distortion and reduction of process of generalization, as well as the depletion of emotional thinking. Thus in patients with paranoid schizophrenia cognitive deficit are more pronounced than in patients with schizotypal disorder. At the same time profiles of psychological adaptation of patients with paranoid schizophrenia and schizotypal disorder have no significant differences.

Abstract
The article is devoted to the methods of overcoming therapeutic resistance in patients with schizophrenia, in particular, during the replacement of antipsychotic therapy. The issues of lack of common views on the problem of resistance are discussed, and in connection with this - the lack of algorithms for managing patients with these problems. Data on the parameters of resistance determination (residual productive symptoms, side effects of antipsychotics, etc.) are given. Data on pharmacokinetic interactions of antipsychotics and their endocrine side effects are presented. The advantage of using ziprasidone for overcoming therapeutic resistance in patients with schizophrenia is shown.

The article is devoted to the application of derived exogenous choline Gliatilin as a neuroprotectant in recovery period of stroke. Abstract
Given the differences in the pharmacological and clinical effects of antidepressants, criteria for their differential choice in the treatment of depression associated with comorbid medical illnesses are suggested. Based on these criteria and general rules of clinical pharmacology, principles of antidepressant treatment in patients with comorbid medical illnesses aimed to optimize the therapeutic process and to prevent treatment resistance are developed.

Abstract
Objective - to evaluate the efficacy and safety of D-, L- hopantenic acid (pantogam active) on cognitive and anxiety disorders in patients with arterial hypertension. Material and methods. Autors selected 80 inpatients of a cardiology department with the verified diagnosis of hypertension and comorbid cognitive and anxiety disorders (50 patients in the main group, 30 patients in the control one). All patients received standard cardiotropic hypotensive treatment. Patients of the main group received in addition pantogam active in the daily dose from 600 to 1200 mg. Psychopathological and psychometric examinations were conducted, the data were compared with the dynamics of physical parameters (ECG, ambulatory blood pressure monitoring, blood chemistry). The duration of treatment with pantogam activ was 28 days. Results. There was a significant reduction in both cognitive and anxiety disorders in the main group compared with the controls and in patients with a history of stroke. The positive dynamics was observed during the treatment period. Conclusions. Authors support the possibility of using D-, L-hopantenic acid (pantogam active) as a drug of choice with bimodal activity (nootropic and tranquilizing) in the treatment of cognitive and anxiety disorders in patients with arterial hypertension. A rapid (in the first week) significant reduction of cognitive and anxiety disorders during the treatment with pantogam activ was noted.
